PPHI-Balochistan was established in 2006–2007. Pakistan launched the "Primary Health Care Model 1999" (later known as PPHI) in District Rahim Yar Khan in 2003 on a pilot basis — an instant success that reinvigorated the government's confidence in public-private partnerships for healthcare delivery.
Following the 18th Constitutional Amendment, which transferred health to the provinces, the Government of Balochistan renewed its agreement with PPHI-B. Over 913 Basic Health Units and Health Facilities across all 33 districts of Balochistan have since been taken over, making PPHI-B the largest primary healthcare operator in the province.
PPHI Balochistan now operates as a not-for-profit registered company (CUIN: 0084896) with an eminent Board of Directors, aligned with the National Health Vision Pakistan 2016–2025 and committed to achieving the Sustainable Development Goals.

Nearly two decades of dedicated primary healthcare service to the people of Balochistan
The PPHI Balochistan was established in 2006–2007. Pakistan launched a "Primary Health Care Model 1999" (later known as PPHI) in District Rahim Yar Khan in 2003 on a pilot basis — an instant success that not only reinforced the government's confidence in the effectiveness of the project but also gave it hope that the ailing PHC sector could be reinvigorated through a partnership arrangement.
In the following years, the government began contracting out the management and execution of primary health services to the People's Primary Healthcare Initiative (PPHI). The 18th Constitutional Amendment enhanced provincial autonomy by transferring, among other things, the subject of health to the provinces. Having the health sector in its purview and considering PPHI's previous performance, the Government of Balochistan decided to renew the agreement with PPHI-B for delivery of primary healthcare services.
So far, over 913 Basic Health Units and Health Facilities across thirty-three districts of Balochistan have been taken over by PPHI-B under the renewed agreement. PPHI Balochistan now operates as a not-for-profit registered company with an eminent Board of Directors.
Public-private partnerships are increasingly seen as playing a critical role in improving the performance of health systems worldwide, by bringing together the best characteristics of the public and private sectors to improve efficiency, quality, innovation, and health impact. Comprehensive primary healthcare solutions for every community in Balochistan
In 2010, high maternal mortality drew the attention of PPHI-B management to strengthen Primary Healthcare across the country. In response, the Basic Emergency Obstetric and Newborn Care (BEmONC) program was planned. BHU Plus 24/7 BEmONC was implemented in all provinces with financial support of the Federal Ministry of Health, Islamabad.
PPHI-B's first maternal health project identified barriers to maternal health and the feasibility of interventions to reduce pregnancy-related risk and mortality. Recognizing that the major causes of maternal deaths were common and largely preventable or treatable, PPHI promoted a set of essential obstetric care services — including not only emergency care but also prevention and early treatment of complications such as anemia, infection, hemorrhage, prolonged labor, hypertension, and unwanted pregnancy.
PPHI-B has established 39 BHU Plus 24/7 BEmONC facilities by end of 2023. With ongoing support from UNFPA for the last five years, these centers provide:
Telemedicine is the remote delivery of healthcare services, such as health assessments or consultations, over telecommunications infrastructure. It allows healthcare providers to evaluate, diagnose and treat patients using technology such as video conferencing and smartphones, without the need for an in-person visit.
PPHI Balochistan, in collaboration with COMSATS Islamabad, introduced telehealth services in Balochistan. Successful pilot projects were implemented at BHU Shado Band, Gwadar, and BHU Wahdat Colony, Quetta.
Following the success of these pilots, the Government of Balochistan approved and funded four telehealth clinics in Washuk, Mastung, Jaffarabad, and Duki in 2020. Subsequently, the Government approved extension of telehealth services to all districts of Balochistan. In the second phase, nine additional facilities were commissioned by June 2021, followed by a mobile telehealth unit in October 2021. During Phase III, five facilities were commissioned in late 2022, with five more by April 2023.
To date, approximately 34 telehealth facilities have been established across almost all districts of Balochistan, offering specialist gynecology, pediatric and dermatology services. With an all-female panel of doctors, the vast majority of rural female population now has access to quality healthcare with the comfort of consulting a Lady Doctor.

In our ongoing commitment to equitable healthcare, ensuring access to essential medicines remains a top priority. PPHI-B aligns with World Health Organization (WHO) guidelines to optimize the availability and accessibility of vital essential medicines across all 913+ health facilities.
The National Essential Drug List (NEDL) of Pakistan, developed in accordance with WHO guidelines, undergoes regular reviews. The current list, last updated in 2021, encompasses 266 items tailored for different healthcare levels by the Department of Health, Government of Balochistan.
Annually, medicines worth over PKR 240 Million are distributed across Balochistan. In FY 2024–25, PPHI-B adopted a phased approach incorporating items for mental health and non-communicable diseases, reflecting commitment to responsive healthcare provisioning within budgetary constraints.
Most health problems of Balochistan are primarily related to individual, group behaviors and lifestyles. PPHI-Balochistan focuses on preventive and promotive healthcare through health education, communication for behavior change, and community mobilization.
PPHI-B organizes a "Support Group" attached to every BHU. Each Group comprises carefully selected individuals representing important interests — elected councilors, notables, teachers, religious leaders, students, and minorities. The Group meets at least once a month and integrates the BHU with the community.

PPHI-Balochistan celebrated World Hepatitis Day with extensive community awareness activities across multiple districts including Panjgoor. Health education sessions, free screenings, and vaccination drives were conducted at BHUs and community gathering points to raise awareness about hepatitis prevention and treatment.
PPHI-Balochistan, with UNFPA support, completed a comprehensive training program for healthcare workers across the province. The training focused on BEmONC skills, antenatal care protocols, safe delivery practices, postnatal care, and neonatal management — improving quality of maternal healthcare at all BHU Plus facilities.
